There is a multitool system out there that comes with many parts like a kit multitool, but I wouldn’t say it’s a great model. It’s a bit gimmicky and slightly more of a novelty.
I don’t believe there are any higher end functional optiosn out there. At least not yet at the quality I would expect.
People have been trying to get leatherman to offer this for years and they have constantly refused becuase of the labour costs involved, which personally I think is bullshit, because if you put a proper team together it’s no more difficult than assembling their typical models. They probably just don’t want to train a new team on it or create factory space for it because that will still reduce their already high profit margins.

No where that does it in an industrial basis, you may be able to get something bespoke from a knife maker but it’s not a simple process making a multitool. There is a reason even leatherman doesn’t yet offer that option

You want a custom built multi-tool, fine you’ll surely get it somewhere.
Like a Leatherman? No.
If you want to get the best but still stand out of the crowd, you should try someone who does good laser engraving. And after that getting done on your LM, search for someone who does good gold plating, & get both the knife blades gold plated, very carefully.

If you have enough money you can find someone to make almost anything custom. A multi-tool would not be a problem. It would essentially be a combination of custom machined metal parts made from the proper materials for the application and heat treated to give the proper toughness. It may be necessary to have some of the parts forged (shaped while hot which adds strength and toughness) just depends on what you want the tool to do. If functionality is not high on your list instead you want it mostly for the aesthetic aspects like a highly engraved or inlaid sword it may be possible to have it 3D printed in metal like stainless steel and polished or plated to get the look you want. You might even consider modifying a manufactured version by engraving or etching or plating to give one a custom look. Or even welding up and machining portions of it to get a look that cannot be made by engraving. There are a lot of gift and trophy shops that might be able to do the engraving. Something more complex could be done by a custom machine shop I actually know a guy that might take on such a project. Works as a machinist at NASA and has his own shop at home. As for a 3D printed part first you need a 3D CAD model of what you want. Shapeways has a shop where you can contact designers that might be able to help you.

I believe so. Any artisan knifesmith, bespoke toolmaker or similar would be worth approaching with your design!
