Lets first start with the simple one.
The easiest way to hide a knife is to keep it in the sole of shoe.

Now for the one seen in movies
This requires a switch blade that works using a spring mechanism. Once the trigger is hit the spring loads, pushing the blade out.
The shoe must have a sole thick enough for fitting the switchblade apparatus in a comfortable manner. Drill into the boot in order to hollow out the sole. Drill a second hole on the right side, i.e opposite of left , of the boot where you can fit in the plug that can hit the trigger on the switchblade.
P.S: Carrying concealed weapons, including switchblades and shoe-knives , is criminal offence in most countries. So be careful.

Unfortunately, “switchblade shoes” have zero basis in reality. I can say this because I studied and collected exotic spy weapons for over three decades and am friends with former and current intelligence agents from several continents… including a few who are custom knife makers. I also used to collect OTF switchblades so I have extensive knowledge in this area.
In order for a switchblade shoe to be viable, it would require a platform sole at least 2″ thick… it also would need a unique firing lever inside the shoe which could be fired with one’s toe while also being comfortable for daily wear with no chance of accidental firing. Also, the sole must be completely rigid with no flexion whatsoever. And this is why it only exists as a Hollywood prop: no-one would be able to walk in it.
This being said, shoe knives DO exist. Well, boot knives anyway. The only realistic portrayal in film was in “Roadhouse” when the guy was prevented from entering the bar because he had a small blade riveted to the toe of his cowboy boot. Yep, these are fixed blades and very small. Punk rockers and metalheads use to stick razor blade in the soles of their boots and smear them with shoe polish. They were used to draw blood upon kicking someone in the shins and would immediately snap off, making it a one shot weapon. My wife used to stick 3 blades in the toe of each of her Docs before going to shows.
Another notable exotic variant is spurs. Not cowboy spurs (although they have also been used) but a curved spike attached to the heel with leather straps. These require a lot of skill and are rarely seen in America, more of an Indonesian thing. Most people just hold their knives in their hand.

I’d assume it is set on a spring loaded pivot if it swings out or it is just spring loaded if it goes straight out. The shoe would be braced in something like a cage and the blade would have a locking mechanism to prevent it from being pushed back into itself. As for how one would disable the blade I have no clue. Could be anything from a button to the step of the heel.
