I worked on network scripted hour episodics. Every week we had knife fights, gunfights and we blew shit up. In the over 10 years I spent making feature film and network episodic, I never once saw a rubber knife or rubber gun. Plastic knives once in a great while. Dulled metal knives and real guns firing blanks. Knife scenes are usually done by stunt people, with the actors doing the same choreography as stunts. This way we can edit in the actors to make the scene look perfect. Everything possible is done to maintain realism, while keeping everyone safe. Before firing blanks, we have a full cast and crew meeting. I take the firearm from the stunt coordinator and lock it in the open breech position. Anyone is offered the chance to handle the weapon and confirm it is not loaded, although no one ever does. We then show the magazine(s) in the same way, along with the individual rounds, in the same fashion…

Usually (and including the sets I’ve been on) don’t use real knives – while they may be made of metal/steel, they aren’t very sharp and don’t tend to cause any mishaps. However, some sets might, I’m just speaking from what I’ve seen.

Yes and no. In terms of close up shots, they sometimes use a real knife in an effort to show the audience how sharp it is and how screwed the fighters are depending on who has the upper-hand. However these scenes are relatively safe because either the actors are careful enough or are often filmed slower than they appear for more detailed shots of the knife struggle. With it sped up so that it looks like they’re fighting in real time. In terms of wide shots, where they are seen full figured, they use a blunted knife. However, just because it’s not blunted, does not mean the actors are safe from mishaps as even a blunted object could still stab you if the right amount of effort was put into it.

No. They use prop knives.
There are often multiple knife props for the same knife, one realistic and maybe even real for close ups, and then various fake ones used in action scenes. Occasionally you can see that the knife has no edge.
